NASG Membership Renewal by Mail

Click on the link below to view, download, and print the NASG Membership Application form. Payment instructions, and the address to which to mail the application, are shown on the form itself.

Although the form requests all sorts of information, if you are already an active member, the NASG Membership Secretary requests that you provide at least this information when filling out the form:

1. Name (required)
2. NASG Membership Number (required)
3. indicate your membership type (especially if you are changing it)
4. (any changes you want to make to your existing Membership Profile, e-mail, phone, etc.)
5. (skip all the other fields that are still the same as before)
